Promotions And Medals?

How are the medals awarded - i'm playing both the LFN and US sub campaigns, i've reached mission 5 in both - the LFN have promoted me twice and awarded three medals (DO,DC and Victory award) where as the USN has seen fit to promote me once and no medals what so ever.
Are the promotions and medals linked to the story or are they task/achievement based. I've probably (guessing so could be wrong as i haven't got the game up at the mo to check my stats) destroyed more tonnage in the US campaign but don't seem to be rewarded as much, or is it that the good old royal navy (i'm english!!) just dishes out the silverware a little more freely??!!
While different Navies historically used different methods for motivation, we did notice that the British and German campaigns are set up such that it is easier to win the medals. We will be fixing this.
